Phillip Matthew Kendall died at his residence on Sunday, July 29, 2001. He is survived by his mother, Sherri Kendall Collins, brothers, Anthony and Thomas Collins; maternal grandparents, Doug and Patricia Slifer; maternal great grandmother, Patricia Slough; Uncles, William Kendall, Douglas Slifer, Jr., Timothy Slifer; Aunts, Kimberly Slifer, Rachel Slifer and seven cousins. Special thanks to the Children's Medical Center of Dayton and of Cincinnati and to Marti and Keith Noble for their help and care during Phillip's illness.
