John J. Cermak, DDS is survived by his wife, Pamela S. Kingery; his parents, Joseph and Sally Cermak of Chesterland, OH; children, Matthew, Mark, Megan and Michelle Cermak at home; brothers and sisters-in-law, Jim and Pam Cermak of Cleveland; Tom and Allyson Cermak of Highland Park, ILL; sisters and brothers-in-law, Kathy and Steve Bitto and Suz and Brian Valentine both of Cleveland; several aunts, uncles, nieces and nephews.

Dr. Cermak was a graduate of Notre Dame and The Ohio State University and was a general dentist with practice in Tipp City, Ohio.

Services 10:30AM Tues., Oct. 24, 2000 at Ginghamsburg United Methodist Church, Tipp City,OH.

at the Baird Funeral Home, Troy, OH.
